2016 Board Self Assessment

The NOAH Board of Directors Assessment is a tool intended to evaluate the effectiveness of the Board. A self-assessment allows the Board to
step back from routine governance matters and to candidly reflect on how well the Board is meeting its governance responsibilities. It helps the Board discover areas for improvement as well as establish a common understanding of roles and responsibilities.

There are three major areas of assessment in the survey:
1) Health Center Performance
2) Board Performance
3) Meeting Effectiveness

Please rate each question accordingly and provide comments at the end of the survey. Submission deadline is January 11, 2017.
We appreciate your feedback and look forward to our continued success.
