We are excited to share with you the 2016 community impact report for Neighborhood Outreach Access to Health (NOAH). In 1997, the vision to bring high-quality and compassionate health services to the men, women and children of our communities was created. This vision evolved from the partnership with HonorHealth and the many community advocates who identified the lack of affordable and quality healthcare services in our neighborhoods.

Over the years, NOAH has successfully integrated seven health center sites, including two school-based health centers. NOAH is also recognized as a Patient Centered Medical Home. This is a testament to the quality of our services.

We are inspired by the ability to bring much needed services to vulnerable families and individuals. Our dedicated healthcare team goes the extra mile each day to be welcoming, thorough and compassionate as they address health concerns that span a wide range. From the refugee families who deal with the stress and realities of life, to individuals facing health challenges complicated by poverty, our providers and staff bring meaning to “healthcare from the heart.” They are the primary source of healthcare for many.
